Endoscopic Ventilation Tube Insertion For Otitis Media (Glue Ear)

Endoscopic Ventilation Tube Insertion for Otitis media with effusion, commonly known as glue ear, is a prevalent issue in children that can result in hearing loss and potential speech delays. Diagnosis is typically made through clinical examination and impedance audiometry. Timely treatment is crucial to improve hearing and facilitate speech development.

The definitive treatment for this condition is the endoscopic insertion of ventilation tubes. This procedure is performed under anesthesia and usually takes only 3 to 5 minutes to complete. Endoscopic visualization offers superior clarity compared to traditional microscopy. A 4mm zero-degree scope is employed for optimal visualization.

During the procedure, a tympanostomy is made in the anteroinferior quadrant, and any accumulated glue is removed. A 1.14mm Shah vent tube is then inserted through the opening. Patients can typically be discharged on the same day without any specific postoperative precautions.

In the accompanying surgical video, Dr. Akbar Abbas demonstrates the procedure on a 2-year-old child diagnosed with otitis media with effusion. Using a standard 4mm 0° scope, a tympanostomy is created in the anterior inferior quadrant, allowing the fluid to drain. Care is taken to avoid injury to the canal walls.

Once the ventilation tube is in place, further audiological testing may be conducted to assess any sensory components of hearing loss. The insertion of the tube is a relatively straightforward process, and no special precautions are necessary afterward.

Despite initial reluctance from parents, many find the procedure highly beneficial for their children. Within a week, improvements in their child’s responsiveness to commands and sounds are often noticeable. The ventilation tubes typically fall out on their own within a few months, requiring no additional intervention.

In conclusion, endoscopic ventilation tube insertion is a safe and effective treatment for otitis media with effusion in children, offering rapid relief and facilitating better hearing and speech development.
