The diagnosis requires a thorough evaluation by an otolaryngologist, and treatment involves a multidisciplinary approach that includes medication and, in some cases, surgery. Step 1: Patient preparation
We have positioned the patient in a supine position and administered general anesthesia to ensure their comfort throughout the procedure.
Step 2: Insertion of the endoscope
To start, I am inserting the endoscope gently through the patient’s nostrils. The endoscope allows us to visualize the nasal passages and sinuses on a screen.
Step 3: Visual examination
As the endoscope reaches the sinuses, I am carefully examining the extent of fungal involvement and checking for any structural abnormalities or polyps.
Step 4: Removal of fungal material
Now that we have identified the fungal debris within the sinuses, I am using special instruments like microdebriders or suction devices to delicately remove the fungal material. This step is crucial to eliminate the source of inflammation and infection.
Step 5: Addressing anatomical abnormalities
In some cases, structural abnormalities or polyps may be contributing to poor sinus drainage. I am using the endoscope to address these issues, removing polyps, enlarging narrow passages, and optimizing sinus ventilation and drainage.
Step 6: Nasal packing and post-operative care
Following the procedure, I may place nasal packing briefly to support healing and reduce bleeding. The patient will receive saline rinses, antibiotics, and corticosteroid sprays to manage inflammation and aid in the healing process.
Step 7: Follow-up care
Once the surgery is complete, the patient will require regular follow-up visits for monitoring their progress, addressing any concerns, and managing long-term allergic fungal sinusitis. This may involve allergy testing, continuing medication use, and lifestyle modifications.
