Hemostasis Heroics: Minimizing Blood Loss in a “Fast and Flawless” Excision of Carotid Body Tumor

Ah, a complex case indeed! This 35-year-old gentleman presented with a Shamblin grade 2 carotid body tumor (paraganglioma), a notoriously tricky mass due to its proximity to critical vasculature. His previous embolization attempt for the tumor, where middle cerebral artery was accidentally embolized, unfortunately resulted in an infarct – a worrying complication. Understandably, his family hesitated to consent to another such procedure.

But fear not! Our “Fast and Flawless” ENT team, accustomed to high-stakes maneuvers, decided to forge a different path. We meticulously dissected, identifying and securing the tumor’s feeding vessels – the superior thyroid artery and the ascending pharyngeal artery. Precision was paramount, as any misstep could compromise blood flow to vital structures.

With meticulous technique, we meticulously mobilized the tumor away from the treacherous internal jugular vein and carotid arteries. Tension hung heavy in the air, but our focus remained unwavering. And then, the moment of triumph: the entire tumor, in all its complexity, was excised in one swift yet masterful move, clocking in at an impressive 58 minutes!

Even more remarkable? Blood loss was minimal, a mere 50ml – a testament to our team’s meticulous hemostasis. No transfusion was needed, further reducing potential complications. And the best news? Our patient emerged from surgery neurologically intact, all cranial nerves functioning flawlessly. His recovery was swift, allowing him to return home the very next day – a joyous occasion for all involved.

This case highlights the importance of collaboration, meticulous technique, and a touch of daring when tackling complex challenges. While the previous embolization attempt presented a setback, our team’s expertise and swift action ensured a successful outcome. Alhamdulillah, indeed! This case serves as a reminder that even in the face of unexpected complications, innovation and teamwork can pave the way for exceptional outcomes.
